I have read and understood the Disclosure Statement related to the Law on the Protection of Personal Data No. 6698, announced by İzmir Tınaztepe University Oral and Dental Health Application and Research Center. I have been informed about the processing of my personal data by İzmir Tınaztepe University Oral and Dental Health Application and Research Center within the purposes outlined below.
In this regard, I give my explicit consent for my personal data to be processed in compliance with the Law on the Protection of Personal Data No. 6698 by your institution. The İzmir Tınaztepe University Oral and Dental Health Application and Research Center processes your personal data in accordance with Article 20 of the Constitution and Article 4 of the Law on the Protection of Personal Data No. 6698, as well as the Health Services Basic Law, Private Hospitals Regulation, Private Health Insurance Regulation, Medical Laboratories Regulation, Regulation on the Quality and Safety of Human Tissue and Cells and Related Centers, Law on Organ and Tissue Procurement, Storage and Transplantation, Regulation on Processing Personal Health Data, and Patient Rights Regulation. The Center also processes your data in accordance with other applicable legislation to which it is subject as a healthcare provider.
Your personal data are retained for the periods stipulated by relevant legislation. They are collected and processed to ensure the provision of the best healthcare services and to fulfill the Center’s contractual and legal obligations. Personal data may vary depending on the healthcare services provided and can be collected verbally, in writing, physically, or electronically through call centers, websites, online services, and similar means.
